Central to CSCGP’s mission is ensuring that all people impacted by cancer have timely access to information, resources, and support from the moment of diagnosis and through end of life. Anchored by the New American College of Surgeons Commission on cancer accreditation standards, our Social Workers empower patients, caregivers, and families and assist them with managing the social, emotional, practical, and financial barriers to care.
This support ensures timely access to quality individualized education, assistance, and advocacy through all phases of an individual’s cancer experience.
We achieve the right level of support by offering timely distress screening at pivotal points along the disease continuum to identify problems before a crisis occurs, encouraging patients to voice their concerns, gain information, and improve the use of healthcare resources.
- Increase care plan adherence through financial, logistical and emotional support, resources, and referrals. We are not YOUR Oncology social worker; as such some referrals for financial assistance will need to come directly from your healthcare treatment team.
- Identify and reduce cancer-related distress in patients and caregivers.
- Increase patient engagement through patient-centered communication and shared decision making. Open to Options™ supports patients and family members as they identify important questions about treatment options and concerns based upon personal needs.
- Assess for barriers to care and provide interventions to improve access and timeliness to care.
- Address social determinants of health and promote health and health equity.
- Ensure survivorship and advance care planning.
